Estimating disaster?

It is probably the nightmare of any project manager. All that can go wrong actually goes wrong…. Unfortunately, it happens, still. At the same time, reflecting upon disasters offers us great learning possibilities. Please check the video below and look for causes of the apparent failure. Do you recognize any obstacles that could have influenced the estimating process, either for time or cost estimating?
